Brookfield, WI — Jay W. Tichelaar, 51, died Monday, May 22, 2017, when the vehicle he was driving was struck by debris from a semi truck.

Wisconsin State Patrol says it happened on I-94 near Johnson Creek shortly after 5 p.m.  The investigation shows a piece of a semi's brake system came off and went through the windshield of a vehicle hitting 51-year-old Jay Winton Tichelaar in the head. Tichelaar died at the scene.  

Authorities are still searching for the semi involved incident but have yet to locate the truck or the driver. The investigation is ongoing.

Jay's wife, Joleen, is very upset with the way the WI State Police are (mis)handling the investigation.  A detailed news article was just published in June 2018 that also contains photos of the accident and has lots of details not contained in the original May 2017 article that is linked above.

The bottomline is that the State Patrol crash report completed by Samborski stated that the parts collected at the scene shortly after the crash were shown to a mechanic who said they were common brake parts used in almost all tractor-trailers trucks after 1999. They couldn’t be traced back to a specific truck manufacturer.  And he interviewed three witnesses, though none could provide a license plate number or detailed description of a truck. One man said he was traveling east on I-94 behind a semi when the truck experienced what he thought might be a tire exploding and he recalled driving over metallic debris. He couldn’t give much of a description other than it was a white truck covered in dirt.

How very random, and how terribly tragic.
